PROGRAM DATES:
18 June – 1 August, 2013-- 9:00 A.M. – 1:00 P.M.
TUESDAYS, WEDNESDAYS & THURSDAYS
For children ages 1 – Kindergarten completion
REGISTRATION & SUPPLY FEE: $50.00
Nonrefundable and due at the time of registration
7 WEEK PROGRAM FEE:
1 CHILD 375.00 (2 Payments of $187.50)
2 CHILDREN 525.00 (2 Payments of $262.50)
3 CHILDREN 675.00 (2 Payments of $337.50)
Children furnish sack lunch daily. Snacks supplied by CHIGGERS.
PAYMENTS:
1. Payments should be placed in the labeled box mounted on the wall in the main corridor of Ed building.
2. When paying cash, please label the envelope with your child’s name, the amount, and the reason for the payment.
3. Please make checks payable to LHUMC-CHIGGERS indicating in the memo section your child’s name.
4. Do not drop off tuition in the church office or give to your child’s teacher.
5. If you have any problems concerning payments, do not discuss this with your child’s teacher, please place a note in the payment box or call Teresa Newsome at 265-8485.
6. There will be a $15.00 fee assessed on any returned check.
