Pick up is anytime between
8:45 PM and 9:30 PM

Registration is late after Wednesday, May 30, 2012  at 9:00 PM.  After that date, we charge a late fee of $5.00 per child, making the cost $15 per child with a maximum of $30.00 per family.

To register, pick up a registration form at Fusion or the Transmitter Campus and submit with payment.  To register and pay online click here.

Each child/family must complete an event waiver form and turn it in upon arriving at Fusion Fitness Center on June 1, 2012.  To make check-in for Parents Night Out easier and quicker, this form can be downloaded at fusionfitnesspc.com/membership or you can complete it the day of event.
